Homeward Bound

Homeward Bound is Symphony at Midway’s state-of-the-art rehabilitation program. It is a comprehensive short-term physical rehabilitation program designed to return residents to their optimal functional level.

Our team of highly trained physical, occupational, and speech therapists work with our nurses and your doctor to create a personalized plan that addresses your needs, monitors your progress, and gives you the support you need for a speedy recovery.

The resident’s care plan focuses on the individual, their goals, and outcomes through the use of nautilus equipment, strength training and pain management. Our therapy team works closely with an expert physician specializing in physical medicine.

Orthopedic Rehabilitation

Symphony at Midway offers a comprehensive orthopedic rehabilitation program that provides our guests all the therapeutic, clinical and technical resources needed to address a wide range of orthopedic conditions, including:

  • Joint replacement
  • Fractures
  • Cervical and lumbar injuries
  • Arthritis and osteoporosis
  • Amputations

Using a team approach to care, we utilize individual therapies, pain management, strength, balance and mobility training, biomechanical re-education, joint protection and other techniques to restore our guests to their optimum level of functional independence.

Cardiac Rehabilitation

Our cardiac rehabilitation program promotes optimum recovery and health for those recovering from heart disease and/or cardiac-related surgery. The goal of the program is to limit the physical effects of cardiac illness and improve cardiovascular fitness and overall health. Each of our guests receives a program designed specifically for them, supervised and implemented by a team of cardiac specialists, including physicians, nurses, physical and occupational therapists, and consulting nutritional and psychological professionals.

Specialized Wound Care

At Symphony at Midway, a holistic plan of care is designed for our wound patients. Our staff is trained to anticipate and care for their needs.

A nurse coordinator, assigned to each patient, works closely with family and physician to monitor progress and serve as a resource to address concerns. They collaborate with the discharge physician’s hospital to ensure appropriate follow up care is coordinated upon discharge from Symphony at Midway.

Our interdisciplinary team, Physician Specialist, Wound Ostomy Nurse, Dietitian, Social Worker, Physical Therapist, Occupational Therapist, Speech Therapist and nursing staff assess and reassess the plan of care. This has reduced our average recovery time and stay by 2 to 8 weeks.
